Statistical Issues on Agreement Analysis

The authors evaluated the value and possible benefit of conventional digital subtraction angiography (DSA) and combined C-arm computed tomography (CACT) of the pulmonary arteries in the diagnostic work-up of patients with suspected chronic thromboembolic pulmonary hypertension (CTEPH). They used Cohen’s kappa to determine the intermodality and interobserver agreement and to assess the correlation of DSA consensus (DSAcons) and CACT consensus (CACTcons) with the standard of reference (SOR) to evaluate the impact of each modality on the final diagnosis. We congratulate the authors on their research but raise concerns about the use of Cohen’s kappa to estimate the agreements.
